*Hospice Nurse Practitioner/Advanced Practice Nurse -- Minneapolis, MN*

The Park Nicollet Palliative Care Team seeks a Hospice Nurse 
Practitioner/Advanced Practice
Nurse to work in collaboration with all hospice care team members by 
delivering hospice
clinical management and related health care services to patients 
diagnosed with complex
symptomatology, psycho-social support relating to terminal conditions 
and their families and
loved ones. Advance Practice Certification required. Palliative Care or 
Hospice education or
experience preferred.
