Crown Health "Medical House Call Service" is a leader in the mobile 
geriatric primary care
service industry. Our providers are second to none and have made 
monumental differences in
the lives of underserved, fragile, homebound geriatric patients. We are 
an innovative team who
is looking to help change the face of healthcare by promoting 
value-based care approaches
that lend to better overall Population Health. We are helping to 
decrease the amount of
readmissions to the Inpatient setting and Post- Acute setting by 
building and fostering tight
knit relationships with multidisciplinary providers, patients, family 
members and care staff.
Our goal is to promote better care coordination between specialties 
which will lend to better
continuum of care for patients.

We are looking for a part-time or full-time Nurse Practitioner to join 
our great group of Board
Certified Nurse Practitioners. The right provider will assist in case 
managing primary care
services for homebound/vulnerable patients who live in Adult Family 
Homes, Assisted Living
Facilities, Memory Care Units and Private Homes. The provider will also 
conduct wellness
exams for homebound patients in King and Pierce counties. There is a lot 
of autonomy in the
position and you'll never have to be inside the four walls of a clinic. 
The right person will have
a passion to do what’s right for vulnerable populations and will have a 
desire to help keep
patients healthy in the community. A full-time position is considered 
3-4 days per week in this
role and the compensation is very competitive. You help design the 
schedule that works for
your life. We utilize an EHR and electronic fax system which makes our 
clinic virtually
paperless. We have an amazing office team there to support the Nurse 
Practitioners and
help facilitate orders and care coordination for patients.

Desired Qualifications: The right NP will have 1+ years of experience. 
New grads who have
experience working with elderly patients will also be considered. Must 
have Master’s degree
from an accredited University, Active WA state NP license, Active 
National Certification and
Active DEA license to prescribe in WA State. Previous credentialing with 
Medicare and other
insurance companies is extremely helpful.

Compensation: DOE. Above average income, Health benefits and other
