*The Community Health Center (CHC)* offers comprehensive, community 
based, preventive,
primary care and wellness services for New Jersey residents of all ages 
at four convenient
locations. CHC health care services are designed to meet the specific 
needs of the entire
family – from annual check-ups and immunizations to sick visits, mental 
health counseling
and critical prenatal care.

We are currently looking for a Family Nurse Practitioner for our Primary 
Care Center, to join
our team. The position would be responsible for providing and 
supervising the quality and
quantity of health care services rendered to clients of all age groups 
at the Primary Care
Center.

1.  Licensure as an Advanced Practice Nurse in NJ
2.  Master’s degree in Nursing from an NLN accredited program
3.  Certification as a Family Nurse Practitioner
4.  Business or management degree or experience preferred 
with demonstrated leadership
skills
5.  2+ years of primary or ambulatory care nursing experience preferred 
6.  Car and valid
driver’s license
