*Position Opening: Subacute ARNP
Locations: Various across Puget Sound *

Group Health ARNPs provide medical management of patients during 
skilled/Medicare Part A
or subacute stays in skilled nursing facilities in a team based model. 
We have established
partnerships with a select number of facilities in Puget Sound to 
achieve excellence in quality
of care, patient/family satisfaction, and clinical and utilization 
outcomes (including
rehospitalization).

The ideal candidate will have the following skills and abilities:
- Experience in advanced practice with knowledge/ability to accurately 
assess through physical
exam, discerning normal variant from abnormal findings.
- Ability to navigate medical complexities through critical thinking and 
analytical skills. Knowing
when to consult in complex medical decision making. Anticipatory 
management of health
status over time, including end of life, palliative care concerns.
- Collaborative practice style, with accountability for professional 
practice within a shared
practice.
- Leadership capabilities within a multidisciplinary team, with systems 
thinking and change
management skills.
- Effective relationships with patients, family, care team, specialists, 
with a patient centered
approach which embodies empathy, respect and collaboration.

*Position Opening: Long Term Care ARNP
Locations: Various across Puget Sound *

Group Health ARNPs provide medical management of multiple medical 
problems and
common geriatric syndromes for residential or custodial patients living 
in skilled nursing
facilities. Professionals who find this job rewarding enjoy working 
independently and
collaboratively with other members of the health care team including 
MDs, nursing, social work,
rehab, etc. to support the preferences of patients and their families 
for preventative, chronic,
and acute care.

The ideal candidate will have the following skills and abilities:
- Assessment and treatment of illnesses frequently experienced by 
elderly patients
- Communication with patients, families, caregivers to achieve the 
optimal plan of care based
on patient/family choices
- Familiarity and comfort with accessing information and documenting in 
an EMR
- Prioritization of patient care needs
- Organization of work responsibilities

community based services
