Palliative Care Nurse Practitioner

Job Description

*

*Additional Responsibilities:*
1. Work in collaboration with the Palliative Care Director and other 
Palliative Care team
members to deliver palliative care and related health care services to 
patients and families
2. Initiate appropriate emergency care in the inpatient and outpatient 
setting until the arrival of
the attending physician.
3. Manage discharge of patients with appropriate disposition, follow-up 
planning, adjunctive
therapy, management, and education.
4. Make assessment of emergency conditions and in conjunction with 
attending physician,
implement management.
5. Perform as independent and interdependent member of the health care 
team providing
palliative care services.
6. Collaborate with physicians and other health care members to assess 
patients to determine
qualifications for palliative care or hospice services.
7. Provide education to patients/families concerning goals related to 
palliative care/hospice.
8. Work with home care/hospice to coordinate a safe discharge for the 
patient.
9. Complete appropriate documentation of all evaluations, care, and 
services rendered in the
medical record.
10. Make appropriate referral for consultations.
11. Respond to telephone inquiries of patients and provide appropriate 
medical care, referral
for consultation or counseling.
12. Communicate with referring or Primary Care Physician regarding 
patient care and
treatment.
13. Provide follow-up and health maintenance care including the 
appropriate adjustment of
medications in accordance with established protocols or in response to 
specific instructions
from the attending physician.
14. Prescribe medication to accomplish the plan of treatment.

*Additional certifications required or preferred:*
- BLS certification required

*PHYSICAL REQUIREMENTS:*
1. Dexterity and eye/hand coordination to perform activities, which 
involve holding, positioning,
manipulating, etc.
2. Visual ability to monitor instruments, distinguish characters 
(letters, numbers, etc.), observe
physical characteristics, etc.
3. Hearing ability to recognize sounds, sound patterns and/or other 
auditory stimuli. Physical
ability to react/respond quickly in emergencies, including life 
threatening situations.
