The Henry M. Jackson Foundation for the Advancement of Military Medicine
Inc. (HJF) is seeking a */Nurse Practitioner/* to support the
Integrative Cardiac Health Project (ICHP), located at the Walter Reed
National Military Medical Center in Bethesda, Maryland. HJF provides
scientific, technical and programmatic support services to ICHP.
*_Responsibilities:_*
1. Identifies and manages outcomes for own case load of patients with
coronary artery disease or with risk factors for coronary disease, such
as hyperlipidemia, diabetes inellitus, and hypertension. Obtains and
records a health history and review of systems. Performs physical
examination for related body systems and interprets abnormal and normal
findings. Orders and interprets related laboratory and diagnostic
tests. Discriminates between normal and abnormal findings in order to
make medical diagnosis regarding physical, emotional, or social
disabilities. Develops a strategic plan for therapeutic intervention
using the patient's problem list.
2. Understands the interconnected nature of CV risk factors and their
impact on the whole person. Provides individualized education to
patients regarding the disease process, diagnostic tests, therapeutic
regimen, and methods of adapting medication and treatment regimens into
the patient's lifestyles. Provides support and assistance in coping
with their disease process. Provides counseling to patients for the
prevention of illness and maintenance of health including; diet,
exercise, sleep improvement and methods for stress management to enhance
overall wellbeing and health. Collaborates with CPP clinical team to
ensure beneficiary is integrated into services provided by the project.
Collaborates with physicians regarding medical care needs of patients,
as well as decisions regarding medical care needs. Monitors patient's
responses to therapeutic regimens and adjusts regimens with established
protocols, recognizing when to refer the patient to the physician.
3. Collaborates with clinical team at Clinical Review, at least monthly,
in the management of patients’ in evaluating and revising therapeutic
plans. Teaches interactive lectures in educational workshops once
monthly. Receives referrals from other health care professionals.
Documents in patient record alt observations, plans, treatments,
teaching, and other important findings. Identifies components of the
regimen that may be delegated to other clinical staff.
4. Works with direct supervisor, Director of Clinical Programs, to
develop and implement NP-managed, outcomes-focused programs in the
prevention of cardiovascular disease and advanced cardiovascular risk
factor management.
5. Participates in ongoing peer review of records with other nurse
practitioners, review of journals, etc. Able to learn how to conduct
exercise stress testing. Initiates emergency care as appropriate.
Maintains current licensure/certifications in appropriate state,
territory or the District of Columbia.
6. Monitors exceptions, deviations, and differences from the established
protocols to identify and report problems. Variances are analyzed and
reported through a continuous process improvement channel to use
solutions and improve delivery of care.
7. Responsibilities include the following: a. Tracks and analyzes
patient outcomes using clinical metrics while monitoring lifestyle
interventions.
8. Develops, reviews, and appropriately updates policies, protocols,
standards of practice, standard operating procedures, and scope of
practice regarding clinical process and advanced cardiovascular
prevention in collaboration with the multidisciplinary members of the
health care team.
9. Maintains an updated clinical database to optimize data collection
and maximize patient outcomes tracking.
10. Serves as clinical consultant to other interdisciplinary health care
providers on the ICHP team.
11. Maintains relevant patient education materials, safety guidelines,
and patient satisfaction system.
12. Ensures risk management measures are fully implemented, adequate
environment of patient care and emergency preparedness, and
competency-based performance of ancillary staff members remain updated.
13. Maintains infection control measure, policy, and activities.
14. Monitors the ICHP patient care related to performance improvement
activities.
15. Assists in preparation for JCAHO inspections.
16. Strives to be a productive, energetic and effective team player.
17. Coordinates or conducts in-service education programs for other
health care professionals. Supervises and instructs students in
clinical experiences.
18. Facilitates and maintains open communication link with related
hospital clinical departments in the event of participant
hospitalization or outpatient procedures, receives participant status
updates and communicates formation back to fee clinical team.
*_Required Knowledge Skills, and Abilities:_* Registered nurse
practitioner licensed from the appropriate state board of nursing;
experience in cardiology or cardiovascular nursing is preferred. Must
have adult education skills, excellent organizational and communication
skills and the ability to work in a participative team-oriented environment
*_Minimum Education/Training Requirements:_* Masters in Nursing, which
includes formal training as an Adult Nurse Practitioner or Family Nurse
Practitioner. Current certification in BLS required; certification in
Basic Cardiac Life Support preferred. Registered Nurse Licensure in the
District of Columbia, a state or territory of the United States and
Certification as Adult or Family Nurse Practitioner
*_Minimum Experience:_* 2 to 4 years clinical experience as a nurse
practitioner and certification from the appropriate national licensing
agency. At least 1 year experience as an Adult or Family Nurse
Practitioner, experience in cardiology or related specialty preferred.
*_Supervisory Responsibilities/Controls:_* Work is reviewed and
evaluated on the basis of demonstrated effectiveness in meeting
program's objectives, initiatives employed, and soundness of judgment
exorcised. Receives supervision from the Director of Clinical Programs,
ICHP, who provides guidance and instruction with regard to operational
concepts, standards, processes, objectives to be achieved as well as
clinical oversight The Medical Director, ICHP, will provide overall
clinical oversight for all patients in the CPP.
*_Physical Capabilities:_* Work includes walking, standing, bending,
and carrying light items.
*_Work Environment:_* Office and clinic setting. Work involves normal
everyday risks or discomforts typical of offices, meeting rooms, etc,
which are adequately heated, lighted and ventilated.
