Winona Health is seeking a Nurse Practitioner with experience in 
geriatrics, palliative care and
hospice to join our Nursing Home and Hospice team.

* Full-time, employed position
* Conduct nursing home rounds at assigned nursing homes weekly
* Assist with systematic development and implementation of treatment 
protocols for nursing
homes
* Act as a resource person in matters pertaining to geriatric care
* Function as a support to the Hospice Medical Director by completing 
face-to-face visits, taking
second tier call from Hospice staff regarding medication management 
issues and symptom
management
* Visit potential palliative care clients
* Complete other support functions to assist the medical director and 
hospice/palliative care
staff

Requirements:
* Successful completion of a Masters in Science degree in Nursing/Nurse 
Practitioner
accredited by the Commission on Collegiate Nursing Education (CCNE);
* Maintenance of an active Minnesota APRN license;
* Current certification as a Nurse Practitioner by the American Nurses 
Credentialing Center
or equivalent;
* Current certification in ACLS.
