*OBOT Nurse Care Manager *
*Program Summary*
The Office Based Opioid Treatment (OBOT) program offers medication
assisted treatment to
adults with opioid use disorders in a medical clinic setting. The OBOT
program is a pilot
program funded by the federal Substance Abuse and Mental Health Services
Administration
(SAMHSA) that was awarded to Washington State in the effort to better
address the current
opioid use epidemic. The program is designed based on the highly
successful OBOT
program utilized across Massachusetts. It combines the expertise of the
OBOT program
medical staff with the clinical backup available from a federally
certified opioid treatment
program for patients who are unstable in the framework of weekly OBOT
program visits. If
successful, the OBOT program is expected to be replicated and
disseminated statewide.
*Job Summary *
The OBOT Nurse Care Manager provides skilled nursing services to opioid
dependent
patients and their significant support systems. Assessment,
intake/admission, evaluation,
triage, teaching, medication induction and stabilizations, and
monitoring of patients in office
based opioid treatment program. Collaborates with OBOT team members and
outside referral
sites, works with providers from ETS Opioid Treatment Program on
unstable patients, with
other addiction and mental health programs and primary care providers in
assisting patients
and supporting their referral needs.
*Job Functions Include:*
1. Education and assessment of patients looking to access opioid
treatment programs.
2. Assessment and intake, including social, medical, mental health,
substance abuse and
medication history.
3. Review consents and treatment contracts with patients and significant
others, obtaining
signatures and giving patient with copies.
4. Review intake assessment with OBOT Program Physician and establish
appropriate plan of
care for patient. Provide and facilitate outside referrals as needed.
5. Assessment and monitoring of patients in the induction,
stabilization, and maintenance
phases of treatment.
6. Use of the Washington State Prescription Drug Monitoring Program
database.
7. Ongoing review and revision of patient’s plan of care with the OBOT
team as needed to
address patient needs and issues.
8. Ongoing management of patients receiving OBOT program treatment
medications.
9. Assessment of signs and symptoms of opioid withdrawal using the COW
(Clinical Opioid
Withdrawal) scale.
10. Ongoing education and support of patients in all phases of treatment
including nursing visits
scheduled and random, urine toxicology screens, routine labs and as
needed, medication
teaching, monitoring, pill counts, observed dosing, and medication refills.
11. Follow State and Federal guidelines in providing care to opioid
dependent patients in
collaboration with licensed, prescribing physicians and pharmacies.
12. Complete appropriate documentation with excellent record keeping.
13. Collect pertinent data using appropriate assessment techniques.
14. Prioritize data according to the patient’s immediate conditions and
needs.
15. Involve the patient, significant others and appropriate health care
providers in the collection
of data when appropriate.
16. Consistently use clear concise and effective written/oral
communication and complies with
departmental policies when sharing/documenting relevant patient care data.
17. Provide follow up care in person and via telephone when needed.
18. Work with the pharmacy and the OBOT team in obtaining medication
orders, prescriptions,
refills, and communication as needed.
19. Collaboration, communication, and meetings with ETS Opioid Treatment
Program (OTP)
team as needed and required for patients who may need OTP services.
Assist patients with
access to ETS OTP intake process as needed.
20. Linkage to other community services including addiction, mental
health and primary care as
needed.
*Qualification Requirements
Education*
Position requires current license to practice as a Registered Nurse in
the State of Washington.
Certification in related specialty areas is preferred.
*Experience*
1. Leadership experience with the ability to work autonomously
2. Minimum of five (5) years of nursing experience preferred
3. Addiction experience required
4. Experience in HIV/AIDS related medical/social issues is desired
*Knowledge Requirements*
1. Knowledge of community and health resources, particularly
addiction-related resources
and supports
2. Intermediate knowledge of Microsoft Office suite
3. Effective verbal, written, and interpersonal skills with ability to
interact with providers and team
members from various settings
4. Sensitivity to ethnic, cultural, gender; beliefs, and behaviors
5. Ability to work in a fast paced environment, with excellent
communication skills, critical
thinking, and problem solving abilities
*Language Skills*
1. Ability to read and interpret general business correspondence,
policies and procedures,
referral information, financial documentation, and applicable government
regulations.
2. Ability to write business letters, uncomplicated reports,
instructions, and procedures.
3. Ability to present information effectively and respond to questions
from patients, staff, referral
sources, and the general public.
4. A bilingual skill set is highly desirable.
*Mathematical Skills and Reasoning Ability*
1. Ability to solve practical problems and deal with a variety of
concrete variables in situations
where standardization may be limited. Ability to interpret a variety of
instructions furnished in
written, oral, schedule, or diagram format.
2. Basic math skills.
*Physical Requirements*
1. The employee is regularly required to sit, use hands to finger,
handle, or feel objects, use
tools, or controls, reach with hands and arms, talk or hear, frequently
required to stand, walk,
and kneel, occasionally to climb, balance, or stoop; rarely to crouch or
crawl.
2. The employee must occasionally lift and/or move up to 30 pounds.
Specific vision abilities
required by this job include close, color, and peripheral vision and the
ability to adjust focus.
The noise level in the work environment is moderate.
*Equipment Used *
Computer, photocopier, fax machine, phone, and medical and dispensary
equipment.
