*Palliative Care- Yakima, WA*

Yakima Valley Memorial Hospital is seeking a Palliative Care Nurse 
Practitioner. The position will entail providing specialized 
age-appropriate individualized consults, assessment, planning, 
implementation, and evaluation of health care for patients from a 
diverse range of cultural backgrounds, and which reflects the 
philosophy, goals and objectives of nursing services and YVMH. The ideal 
candidate will be able to work independently in home settings 
coordinating care with caregivers and family members, to use teach back 
skills and other tools to engage the patient and family in goal setting 
and self-management, to complete a comprehensive/focuses assessment of 
the patient health condition, symptom management interventions and 
develop goals in active collaboration with patient’s attending 
physician, to support difficult decisions for patients/families such as 
end of life (Palliative Care and Hospice) services and coordinates with 
the Interdisciplinary team in planning and achieving patient goals 
involving the patient and family in the care planning process. We offer
