We're looking for talented Nurse Practitioners and Physicians Assistants
in Colorado Springs, <br>
Pueblo and Canon City to join us in an autonomous role. We offer
orientation to long-term <br>
care, including preceptors and ongoing support. Make a direct impact in
the lives of the <br>
people we serve and enjoy the freedom to set your own schedule when you
apply your <br>
expertise with us. <br><br>
* Primary responsibilities include: <br>
* Making on-site visits to long-term care facilities to provide care to
aging residents <br>
* Managing chronic conditions as necessary <br>
* Working actively and collaboratively with physicians to manage a
caseload of 80-100 <br>
patients on a monthly basis <br>
* Building relationships and maintaining positive communication <br>
* Setting up appointments and managing a caseload <br>
* Making rounds with physicians <br>
* Participating in care conferences with patients and families <br><br>
The successful candidate will be a licensed NP or PA. National
certification in family, adult, <br>
or geriatric is preferred. Acute care certification is also acceptable.
Long-term care <br>
experience is a plus, as is RN experience. <br><br>
