management type position needs a computer literate, organized, team
player to assist with <br>
enrolling pts (performing initial Hx and PE) and visiting the pts in
their homes periodically. NP <br>
will work in conjunction with primary MD, and the other team members to
manage chronic <br>
diseases and prevent acute exacerbations of such. Candidates should hold
family, adult, or <br>
geriatric NP or CNS. New grads are always welcome and great mentors are
provided! Salary <br>
range is $62-$72K, excellent benefit/health package, and relocation. One
