Practitioner who enjoys serving the geriatric population. Our

living and skilled <br>
care settings in northern Colorado.<br>

<b>Overview of Job Duties:</b><br>
Evaluate and treat patients in assisted living and skilled care
settings.<br>
Make diagnosis; refer patients to other healthcare providers.<br>
Collaborate with PCP's and other Physicians.<br>
Select, evaluate, interpret and follow up on all lab tests ordered.<br>
Complete all health evaluations, summaries and chart notes in EMR
system.<br>
Maintain professionalism with patients, families, co-workers and
facility staff.<br>
Initiate and alternate appropriate medication within established
protocols.<br>
Take call on a rotating schedule.<br>
<br>
<b>Qualifications:</b><br>
Must be Licensed as a Nurse Practitioner in Colorado.<br>
Must hold DEA license/ Prescriptive authority for the State of
Colorado.<br>
2+ years of Practicing Experience as a Nurse Practitioner.<br>
<br>
<b>Schedule:</b><br>
Full-time/Day time hours. 40-hour work-week Monday through Friday.<br>
<br>
<b>Location:</b><br>
Fort Collins, Colorado – Service area in northern Colorado. Fort
Collins is <br>
considered to be one of the most beautiful areas of the county. Home
to Colorado <br>
State University with a 22,000 student enrollment. Fort Collins
boasts a city <br>
population of 140,000 offering multiple shopping, dining, and
entertainment <br>
options.<br>
