<b>Annual Wellness Visit Program opportunity in Oregon’s
Mid-Willamette Valley</b>
<p>The Corvallis Clinic is actively recruiting a certified family
nurse practitioner or certified adult geriatric nurse practitioner
to join our CMS Annual Wellness Visit (AWV) Program. The nurse
practitioner will perform the annual visits or facilitate their
completion with primary care providers. The goal is to increase
the number of completed wellness visits while maintaining the
highest level of patient care.
</p>
<p><b>Responsibilities</b>
<br>
* Complete a comprehensive patient and family assessment to
determine priority needs and, in collaboration with the patient’s
provider and other team members, design and implement an
individualized plan of care.
<br>
* Promote education of medical health conditions. Ensure that the
patient and/or their caregiver are educated on managing their
medical condition and comorbidities with the goal of improving
medical outcomes.
<br>
* Use multiple interventional strategies and opportunities to
assist patients with self-care and symptom management.
<br>
* Collaborate with operations directors and managers to schedule
annual wellness visits at multiple sites and occasionally in
patients' homes.
</p>
<p><b>Qualifications</b>
<br>
* AANP or ANCC certification
<br>
* Minimum one year experience working as a licensed nurse
practitioner; geriatric experience preferred
<br>
* Experience with CMS compliant AWV’s highly desirable
<br>
* Excellent written and oral communication and problem solving
skills
<br>
* Strong customer service orientation
<br>
* Oregon license or eligibility
<br>
* Valid Oregon driver’s license and reliable transportation
