The fact that I am not an MD is occasionally a problem. The bigger problem is that many people only hear "nurse" or otherwise assume I am a nurse (ie-female means nurse even if nurse means MA, NA, LPN, RN, etc). With more individual contact, patients generally accept me well. My role is hospitalist for a cardiology group. 3. I contact my supervising MDs. They round almost daily on all my patients anyway, as I work only in the local hospital at which the group has a very large patient census. 4. Resources run the gamut from books, journals, professional societies, etc. 5. I have been in this role for 8 months, so I am still getting comfortable. My comfort level is vastly improved over that period of time, and my primary MD notes the dramatic increase in my clinical knowledge and skills over that time. 6. I do not think the DNSc is the ticket that our profession needs. We are more in need of stronger MSN training programs with stronger clinical work. I think that MSN is adequate educationally if the program is adequate. The "degree creep" is showing up all over health care, notably the PharmD degree. I agree with a previous responder that we need to focus more on getting RNs educated solely at the BSN level, on differentiating LPN from RN (see above comment about the meaning of "nurse"), etc. Unless nursing mandates BSN as the entry into practice degree for RNs, we will always be a seond class profession. But, that also means making the BSN graduate functional upon graduation, not 6-8-10 months later. Also means having appropriate assistive help so the RNs can be RNs. 7. No experience with contracts. I would recommend avoiding non-compete clauses, although some say that they are unenforceable. Be sure that your salary is commensurate with your productivity in charges and reimbursements. 8. Yes, no question. Although as mentioned by the previous responder, the money may be less than RN pay and the hours are certainly longer. 9. Have adequate clinical (ie-medical/physician/experienced NP) support and a strong mentor in your first job. 10. NO 11. Support, mentor, salary, job growth. 12. Often. You just have to move faster and, unfortunately, reduce your patient education time (I make strong use of our patient ed service). 13. Strengths: prior healthcare experience, broader perspective, ability to communicate on appropriate level with patients/families/hospital staff. Weaknesses: use as MD assistant/gopher 14. Not really my issue. Office staff has to do med precerting often, which is sad but required. I try to use oldie-goldie meds when feasible, but main focus is on documented effectiveness. 15. Not my issue - handled by office staff. But in my previous job, it was a maddening issue. The medical professions have allowed insurance industry and government too much influence in medical decision making. Exceeding excessive amounts of time (translated money) is spent keeping the insurance companies happy. 16. No. Plus PAs are typically paid more for the same job. Again, the association with nursing profession (ie-You're really a nurse) is a hindrance. 17. Patient education. I have to distance myself from RN care lest I be treated as an RN, etc, etc. In any job, be sure that you are not lumped into the nursing part of the organizational chart. This is critical. 18. Building trust is key in the nurse to patient relationship. The question is complex and honestly I have run into struggles in my career...but how you define it can be difficult and in what context are you speaking to in reference to trust? I have had patients that have required more time than others in order to build the trust and working with patients with questionable mental incapacities presents their own challenges. When you are working with a patient who has lost their family and those who come to you with tremendous losses in their lives present challenges as well. Trusting a health care provider following a brutal rape from a patient perspective can also be a challenge...so there are many circumstances where trust is challenged. I do not define these as "struggles" per se. Nursing is an art and science and the delivery with care and compassion overcomes the "struggles," though there will always be challenges. Interestingly enough, I find that NPs have advantages over many other health professionals because they are nurses, not in spite of this. Nurses are very trusted by the population, and this trust seems to extend to NP's as well. I think the majority struggles that I've encountered in my own practice relates not particularly to myself as a person, but rather the fear and mistrust of the health care system, illness, etc. As NP's we sometimes deliver news which is burdensome, troubling and life-changing for patients and their families. Yet, it is within these moments of disbelief and mistrust that a therapeutic and trusting relationship emerges. Has getting patients to trust you been a big struggle? Same as above What do you do when you do not know how to treat a patient? What do you say to the patient? I tell the patient."I don't know what's going on here" I think being honest is the most important aspect to practice. I know that I do not know everything. I regularly use consultants, either other NP's, MD, pharmacists, PT/OT, SLP etc. What resources do you use to keep up-to-date? Journals, NPlinks, Academic clubs, review and participate in clinical practice guidelines, participate and present at conferences, How long did it take you to get comfortable? Comfort can be a dangerous thing. I think you need to know what you know and know what you don't know, and have the wisdom to know the difference. Too much comfort can mean complacency, and in any level of nursing practice, a complacent (we've always done it like this.type nurse) nurse is a dangerous and often out-dated nurse. What is your opinion of the Doctorate of Nursing Practice? Over the years, the educational background of NP's has changed significantly: from post-diploma preparation, to post-bachelor preparation, to the master level of entry. I think the DNP is a safe way to standardize practice throughout the country and the world. It also provides clinicians with the highest level of academic and clinical credential. In the US, NP's are often referred to as "mid-level" clinicians. I feel that this is in fact insulting to nursing practice, that at even the highest level of practice, we can only be "mid-level." Nurses need to embrace their profession and work together to advance the significance of nursing care. What aspects should I look for in a contract? Vacation. Respect. A good salary, don't under-estimate yourself! Are you glad you became a nurse practitioner? Yes. A very rewarding and excellent way to provide clinical leadership in nursing. What advice would you give to a new NP grad? Find a safe and supportive environment to work in. Do you feel you had enough education? Yes. What should I look for in a job? Administrative and colleague support. Money and benefits, education opportunities. Are you pressured to see more patients that you can handle? How do you handle it? Say no. What are the strengths and weaknesses of the NP role? Strengths - I find that patient receive better care. Weakness - haven't found any.
