[NPInfo] that PMR pt

Carla and Dena,
he was in his apt until he finally asked for more help. he was sleeping 24 hrs in a lounge chair. he has a dependent personality, was all alone, and frightened. The asst living was to be temporary until we could control his pain, get him pt/ot, feed him and help with his toileting. he hallucinated with pain meds, mult myleoma was ruled out and PMR was diagnosed by exclusion and reaction (pain relief) to prednisone.

He is a big and tall guy, so it would have been difficult to get him to ct scan and other studies, so he was admitted to the hospital for the work up, and the CA was found. He never had sternal pain, and his back pain had disappeared.

I only met him 3-4 weeks ago. he had spine films done in the spring when he hurt his back helping a lady who fell off a boat. Nothing apparently was seen then-I never saw those reports, just went by what the doc told me.

Sorry back again Paula, not to give you the third degree just trying to find out. You said he was "failing in his apartment" I understand he can get his meals and laundry at assisted living, but was someone there doing his personal care, and staying with him to walk with him and take him to the doctor's visits, and help him dress? Or was he not able to stay in his own apartment where he was comfortable and get a care giver in there, like family, or hired staff, as that is cheaper than the assisted living, and you can get meals on wheels to help with the food issue, and also hospice so there are initial nurses at home with good pain meds and anxiety meds. I would have thought the compression fxs would have been a tip-off to do a
work up with his hx of prostate CA. What was the cause of the compression
fxs? Men don't get them the way women do-- or for the same reasons. In a
pathological fx in a man with hx of prostate Ca I would have followed up the
spine x-day with a bone scan and full mets work-up.

he had prostate ca several years ago and surgey. what is weird i that he had
spine films and chest films-negative except his compression fracture last
spring. He was failing in his apt so we put him in asst living-had a lot of
muscle pain, was incontinent since prostate surgery, was bowel focused and
needed help stooling. The pred nisone we started last week pretty much
eliminated his pain, but his legs were weaker r>l, dev foot drop. that is
when we admitted him. CXR shows a cavitation in a lung, ct scan shows his
manubrium almost eat up (never c/o pain there), a mass in the kidney and
maybe liver, and in his spine!
