I have a book on coding modifiers, and will look this up for you. But yes, I believe if you have a separate e/m code, you certainly can attach it to a modifier and bill it on the same day. Your exam should be for the initial dx and ICD 9 codes, and then a separate exam for whatever you admitted for, or for any separate identifiable problem that required immediate attention that same day.
