Got this from a friend-sounds like a great resource to make customized  forms 
for your practice's website:
 
Just got a handle on this site which allows you to make online forms  to
refer back to you such as if you want patients to fill out forms ahead  of
time or if you were doing house calls you could have them fill the  forms
out, check off their symptoms, send them back to you - you could then  almost
dx the condition before even seeing the patient.  Allows  surveys,
registration, etc.  Uses or boundless.  Don't know the  cost, and coming from
someone who is going bankrupt (and probably having to  go through divorce
because my husband blames me for the bankruptcy) it looks  doable - at least
to take a look:
