[NPInfo] Help - need California hospice

I live in Michigan and have an aunt who lives in California.  Based on my 
discussions today with my cousin, she would be appropriate for hospice care. 
  However, my cousin received poor advice from discharge planners and isn't 
getting much guidance.  Can anyone recommend a hospice in California?  She 
lives in a town called Chino which I believe is near LA.  Thank you.
