[NPInfo] Help - need California hospice

Sue--
A very simple google search immediately brought up several options,
including VNA and Hospice of Southern CA which listed 3 zip codes in Chino
as part of their service area (could Chino have MORE than 3 zip codes???).
They have been in business since 1952 and, according to their website
(http://www.vnasocal.org/index.htm) it looks like they are JCAHO, Medicare,
and Medical certified. The Internet is a marvelous tool for finding stuff.

I live in Michigan and have an aunt who lives in California.  Based on my 
discussions today with my cousin, she would be appropriate for hospice care.

  However, my cousin received poor advice from discharge planners and isn't 
getting much guidance.  Can anyone recommend a hospice in California?  She 
lives in a town called Chino which I believe is near LA.  Thank you.
