[NPInfo] Help - need California hospice

Hi Sue,
   
  I finally heard back from my sister, and she actually has not put anything in place yet, but this one 93 yr old gentleman whose home is near Chino, is staying at St Jude Hospital right now, due to a fall, but they also have an excellent reputation as a hospital and for their hospice. Hope this helps. Of course it is all on paper, but the continuity of care philosophy sounds good.  Carla/Portland Here is the link:
