[NPInfo] Medicare Number

 I just obtained my Medicare number for myself as an individual and for my own clinic that is a corporation. I had an individual one that was different in Washington, and an even different one that was associated with another employer in Oregon.   You can either keep those active if you still work in those places/settings or close them.     The clinic or group also has a Medicare billing number. This gets attached to the tax id number.  Any NP that works for  that clinic will have their own Medicare number, but be assigning it to the clinic or group Medicare  billing number regardless  if the NP is working as an Independent Contractor or Employee. If the NP does not have their own individual Medicare number, initially the Medicare patients are billed under the provider (MD or NP) who owns the practice, or is supervising, but that MD or NP has to be part of the patients care, ie see the patient some of the time, be on site..in other words follow the "incident to" rules..so
 generally, most clinics that are thorough with credentialing, will want the NP to have "their own Medicare number" and then you will be billed under your own name as the provider, but attached to the clinic Medicare number as well.
   
   So again, your second job, should not just "bill" under one group number. If you are going to be seeing Medicare patients and be an independent contractor , you will need your own Medicare number.   For your own Medicare number you fill out an 855i, and for each clinic or group  you work at you fill out an 855r.
