Agree with you Kate.
I do not have PhD on my id at the nsg home where I practice. And never 
address myself as Dr. in the facility as few in this rural southwestern town 
would appreciate the title and the residents would be confused although 
several do call me "Doc". Colorado does protect the title for doctorally 
prepared nurses. But I suspect I may be the only nurse with a PhD within a 
350 mile radius.
.
However, I always refer to MDs, DO's as physicians, never doctors.
