[NPInfo] New grad dilemma

Just based on the context of your letter, these are my thoughts:

- you seem to want to teach so that part appears to be a definite
- as for the other days of the week. . .
	1. you don't seem at all enthusiastic about the NP-run nursing home.
So scratch that off your list.
	2. Retail Clinic sounds like an opportune situation but I understand
the concerns you listed.
	3. The Specialty clinic sounds like a fantastic opportunity that you
are really excited about.

Here are my personal thoughts on your above situation.

I would go ahead and train for the retail clinic. You have no idea if you
will get the position with the specialty clinic or when they would even
notify if you did. If the specialty clinic does offer you the job and you
still feel it is a perfect fit, then you can respectfully resign from the
retail clinic based on a once-in-a-lifetime opportunity.

I'm curious as to what the other members think.

Respectfully,

Linda Marie De Zago, MS, ARNP-C

Subject: [NPInfo] New grad dilemma

I am a new FNP grad, actively interviewing. Hadn?t practiced clinically in
13 yrs until the recent rotations. Prior to that was in ICU/ER, a CCRN. Am
now 50, finished PhD 5 years ago. Tuesday passed my boards, suddenly two
agencies want an answer.

Need flexibility. Will be teaching in my nsg school, considering a career
there, but can't teach NP courses without experience. Commitment is 1
class/week, other part of that day will be doing physicals for foster
children. The rest of the week, have 4 options:

1. Nsg school above- clinicals in hospitals, sporadically 7 -14 weeks at a
time, would make nsg school happy, pay isn't great. If commit to one/two of
the others below, could end up working spurts of 7 days a week.

2. NP-run nursing home agency 2 days/wk at one home; could be 3 days by
adding 2 other homes. Pay ~ $41/hr in billable units- must see approx 16
people for 8 hrs of pay (behavioral health visits can be faster). If not
enough acute problems, need to case find- bimonthly physicals etc. They want
me to sign. Have shadowed, meeting administrator and DNS this week. Seems
ok- pleasant, decent pay, mild pressure, geriatric experience.

3. Famous retail clinic- interviewed this week, wanted me to sign then.
$45/hr with 2-12 hr shifts, 1 weekend/mo. All grads must orient before
Oct/flu season. Anticipate a good deal of down time, could work on student
papers etc. Very seductive, good pay, low professional interaction. Wonder
would I lose knowledge fresh from school that wasn?t in their scope.

4.  Specialty clinic in a famous pedi hospital. They need research
background and interest in alternative medicine, I have both. Two APRNs,
several MDs, seems stimulating. Interview in 10 days, unknown when would
hear back. Pay grade for NPs here is $65-105,000 range. Would be
inpatient/outpatient experience, but more demanding, no adults, no downtime.

Would appreciate your collective wisdom- what would be the best factors to
consider for a first experience. How is future marketability once no longer
new grad. Would working in a retail clinic be considered good experience for
urgent/primary care later on?  How much longer should put nsg home and
clinic off before I lose my edge?

Thanks,
Gail
