[NPInfo] Looking for NP clinical evaluation tool

Does anyone have a clinical evaluation tool they use to evaluate new NP (or
PA) hires and for yearly evaluations that they would be willing to share
with me? We are trying to formalize our orientation process and need a tool
that would help us identify strengths and weaknesses of new hires so that we
can counsel them with specifics. The next step will be to develop a more
formalized yearly evaluation process (currently we have nothing structured
or in writing). We work in a collaborative state.
