When I was at the VA, I worked with "clinical pharmacists" who had limited prescriptive authority. They were very helpful, actually. One pharmacist ran a Coumadin clinic and could order coag profiles labs and titrate doses. They could monitor aminoglycoside peaks and troughs, adjust insulin doses, etc. One pharmacist had a hypertension clinic and was quite an expert at getting poorly-controlled blood pressures down. He was fearless about adding in third or fourth antihypertensive agents. He actually kept a lot of patients out of Cardiology Clinic, which was good. 
 
But the pharmacists couldn't diagnose, and this helped to keep them off our "turf". They saw patients only by consultation from a primary care provider or a specialist.
