For several years I have been measuring Vit D OH 25 levels-  and I was  
taught as a functional medicine practitioner that it was more like a hormone  then 
a vitamin.  It also works with Vit A and if you have sufficient  amounts of 
Vit A you can not overdose on VIT D-- The goals I use is 60-80 and  unless you 
get over 180 you have no problems with toxicity.  I use a high  quality 
pharmaceutical grade formulation of 1000 iu or 5000 iu's   orally.  I started out 
conservatively but have now found that it takes  10,000 a day for a few weeks 
followed by 5000 a week for most people to even get  above 50.  The recent study 
released on Breast Cancer showed that Women  whose levels were less than 42, I 
believe,  were at increase risk.   There  have only been a few documented 
cases of D toxicity and it was in  patients who were taking 100,000 to millions 
of units by accident and even at  that as soon as it was stopped their livers 
recovered and they were fine.   If you get over 180 to 200 than you are pulling 
calcium out of the bones.   Otherwise a very safe and miraculous treatment 
for many issues. I had the  privilege of hearing Dr Hollick who has done most of 
the recent research  on Vit D lecture and receive the Linus Pauling award.  I 
also was  able to talk to him personally and ask many questions.  He 
originally was  almost kicked out of Boston College for his original comments and 
findings and  now is quoted or listed on almost every research article on Vitamin 
D.   They use 2000 iu in Finland in newborn babies and one study showed a 
decrease of  37% in type 1 diabetes in children who received the 2000 iu from the 
day of  birth until the age of 5 over children not given the Vitamin D.

I  have an article coming out in the July issue of Clinician Reviews on this 
very  topic.  Current recommendations:  

OTC Vitamin D3 1,000 IU  daily
Rx: Ergocalcitrol 5,000 IU 2 times a week for 8 weeks 
OR
Ergocalcitrol 5,000 IU weekly for 5 weeks

Retest at 6-8  weeks.

Current treatment goals are serum 25-Hydroxyvitamin D greater  than 30 ng/ml 
although there is research that indicates greater than 50 is  beneficial in 
cancer prevention.

I  have many  pts diagnosed with low levels Vit D (most are "healthy "  women"
under age 50 with aches/pains/ breast cancer but few chronic  disease)  -does
anyone have protocol for treating, monitoring, and  follow up?
