Is this patient on any other medications... for example, long term Dilantin can contribute to high HDL and low WBCs...

Respectfully, however, I thought about the best one could do in lowering
total cholesterol through diet alone was around 10%, which would still leave
her high. What about exercise or adding in niacin during those 6 months?

Thanks for your input!  Her HDL is terrific (which contributes to her high 
total cholesterol) but the concern is the LDL and the non-HDL (at 189).   I
have 
always considered a high HDL very protective, too (and it is)... until I
have 
attended some CME classes that have stressed the importance of the non-HDL, 
even in the situation when the HDL is high. How are you using non-HDL?  
 
My plan was (is) to send her to a dietician and repeat the lipid panel in 6 
months (she is a pharmacist so is pretty aware of treatments).  I will
repeat 
her CBC in a month or so.
