Another source of "body odor" that I have identified (especially in teen
boys/young men) are athletic shoes that they "live in" in the summer, often
without socks. They or their mothers will complain of body odor, and for
some reason, not be able to trace it to the shoes (that are likely full of
pseudomonas). The whole exam room will often stink, so it does make it hard
to trace the source when the odor is so strong.

Dave, Your comment on the clothes is right on target. I "cured" a teen of
her body odor after a discussion about "even if you do not think you sweat,
you do, so wash your clothes often!"
Another comment- For the patient in question, is this underarm type odor or
perhaps an oral health issue such as dental decay or gum disease? Is she
ketotic? The later is common with dieting and can be quite intense.
Bacterial vaginosis? Can the examiner detect the odor?
