I already discussed all of this with my attorney. He does business law for the state of Indiana. In Indiana, all licensed personnel can form corporations. A license statement is sent to the the attorney from the state board to be kept in the corporation's file. This statement can only be issued if a collaborative agreement is in place. You can't be issued a CSR in Indiana without a collaborative agreement. You can prescribe non-scheduled medications and treatments without a collaboration. However, this restricts practice as we all know. There is no medical corporation, nor nursing corporation in the state of Indiana. All licensed individuals and this includes attorneys, psychologists, chiropractors etc. are able to form corporations under the umbrella of a Professional Corporation aka PC. BTW, did you notice that the d/ba for my P.C. is APN, P.C. I love humor!!
