Hi Suellen,
I could not agree with you more. My malpractice went up from $600 to $900. What are you paying? I work in psychiatry. I don't know why the increase either. And I don't get the 'extender' reference either. I usually hear of us as midlevels, like Physician Assistants in some way. My role is very autonomous in outpatient psychiatry. I work very independently and bristle at the idea of being an extension of the physician role. My clients find seeing me more satisfying than the physician. Anyway, just thought I'd join you on the ole soapbox.

Nothing irritates me more than the term "physician extender."
An NP is not an extension of the doctor.
We are practitioners with a theoretical framework, mission and goals.
We are members of a profession.
We are more than a physician!
That term was "outlawed" 5 years ago. How in the the world did it creep back into the literature?
Even the Federal Gov't had finally recognized NP's as a profession unto ourselves and not an extension of any other profession. How is it that we are going backwards?
Have we rested on our laurels too long?
I've noticed that the percentage received in a joint practice is also decreasing as our costs are rising (i.e. malpractice insurance.)
Speaking of malpractice insurance, why did it double when our statistics have not changed? Anyone have any ideas?
In closing, please let us not do ourselves a disservice. Let us not take a step backwards.
We are nobody's extension. We are Nurse Practitioners.

Use of physician extenders can significantly improve your practice's efficiency. However, billing incorrectly for them can cost your practice significant dollars and make you the target of audits from the OIG.
