[PrvPracNP] Do you have a medical director agreement to share?

I am putting together a draft agreement for the doctor who has agreed to be  
the medical director of my hopeful prenatal clinic. I have a few collaborative 
 agreements that some of you have shared with me over the last year or so 
that  have been helpful on what to include. (I will be hiring an NP who will be 
an  employee of the clinic).
 
But the sample agreements don't really address billing with the  doctor's 
provider numbers which we will be doing while we try to apply for our  own. Since 
the MediCal( CA Medicaid) and insurance company checks will come with  the 
doctor's name on them, how will this work with income tax, his independent  
contractor status, etc. My attorney is forming a nursing corporation for  me.
