[PrvPracNP] Does anyone have an arbitration agreement clause to share?

The attorney told me to add a clause concerning arbitration and  attorney 
fees for the contract with my medical  director but I cannot find  one-the 4 
contracts that NPs shared with me don't have anything related to legal  fees, etc.
