Message-ID: I'm hoping to open my prenatal clinic in about 2 weeks (finally!) and am having a hard time finding patient registration forms and health history forms that new patients fill out in Spanish. If you use these in your practice, could you please share them with me? Ideally, I'd like to have 2-3 so I can make up my own. I'm not impressed with the one Medical Arts Press has but I'll buy their HIPAA Spanish form.
