POSITION TITLE: Nicotine Dependence Treatment Counselor/Nurse Practitioner

TYPE: Part-time contractor; minimum of 20 hours per week

CLOSING DATE: Open Until Filled

Job Description

Contractor will provide evidence-based counseling services in support 
of Hollywood Quits, an
entertainment industry-focused smoking cessation program in 
Hollywood, which is funded by
American Legacy Foundation and operated in association with Mayo 
Clinic. During the term of
this Agreement, the Contractor will work part-time in all aspects of 
the program service delivery,
serving primarily as a nicotine dependence treatment counselor. 
Counselor will be trained and
managed by the Mayo Clinic Nicotine Dependence Center.

This role will include the following:
1. Provide intake in-person and over the telephone
2. Triage clients and coordinate care with their primary care 
physician as appropriate
3. Develop individualized tailored quit plans that incorporate 
individual counseling,
      pharmacotherapy, and website and telephonic quit services
4. Conduct in-person and telephone individual counseling and 
in-person group counseling
      sessions
5. Collect patient and program level data and enter it into a 
database on a daily basis
6. Communicate and coordinate day-to-day program management across 
multiple parties
      involved
7. Coordinate pharmacotherapy prescriptions for program patients
8. Conduct telephone follow-up calls to monitor abstinence
9. Administer biochemical verification of cessation with carbon 
monoxide monitoring
10. Assist with evaluation and modifications of the smoking cessation 
program in coordination
      with Mayo Clinic
11. Support the development of collateral materials for the program 
as necessary

Qualifications
- Licensed, independent practitioner
- At least 10 years of clinical work experience; work in substance 
abuse, mental health, and
    individual therapy preferred
- Masters degree in nursing, counseling, psychology, social work or 
related field preferred
- Certified Tobacco Treatment Specialist is preferred
- Demonstrated advanced problem-solving skills and ability to work 
independently
