We're searching for a Nurse Practitioner to provide primary care 
services in a collaborative
environment for frail and elderly patients in a long-term-care 
setting. Our Nurse Practitioner
and a Primary Care Physician form a primary care team that works with 
the nursing home
staff to help maintain the health and well being of our enrollees.

Primary responsibilities include:
* Providing direct care to patients in long-term care facilities
* Driving from facility to facility on a daily basis
* Collaborating with primary care physicians and nursing home staff 
to assess needs and
review plans of care
* Going on rounds with physicians as needed
* Participating in care conferences with medical staff, members and 
members' families (in
person and over the phone)
* Managing a caseload of assigned patients
* Communicating on a regular basis with clinical advisors or service 
managers to review the
caseload, discuss transfers and assess risk
* Providing education/in-servicing to nursing home staff and physicians
* Learning/using the Evercare-specific electronic documentation system

Qualifications:
Only nationally certified NPs (Geriatric, Adult, Family or Acute 
Care) or new NP graduates with
RN experience will be considered.The ideal candidate will have 
experience in a long-term
care setting. Computer proficiency is required.
