We're looking for talented Nurse Practitioners and Physicians 
Assistants in Colorado Springs,
Pueblo and Canon City to join us in an autonomous role. We offer 
orientation to long-term
care, including preceptors and ongoing support. Make a direct impact 
in the lives of the
people we serve and enjoy the freedom to set your own schedule when 
you apply your
expertise with us.

* Primary responsibilities include:
* Making on-site visits to long-term care facilities to provide care 
to aging residents
* Managing chronic conditions as necessary
* Working actively and collaboratively with physicians to manage a 
caseload of 80-100
patients on a monthly basis
* Building relationships and maintaining positive communication
* Setting up appointments and managing a caseload
* Making rounds with physicians
* Participating in care conferences with patients and families

The successful candidate will be a licensed NP or PA. National 
certification in family, adult,
or geriatric is preferred. Acute care certification is also 
acceptable. Long-term care
experience is a plus, as is RN experience.
