The HBPC NP/Program Manager, is a professional registered nurse with advanced
education from a university-based Nurse Practitioner program and 
appropriate experience
who provides evidence-based care for a specific population of 
patients who are managed in
their home. The Nurse Practitioner uses advanced assessment, planning 
and evaluating
competencies, as well as approved standardized procedures, to provide 
family centered,
preventive and illness management healthcare incorporating the 
patient's age, gender,
culture and learning abilities in the treatment plan. The Nurse 
Practitioner works
collaboratively with many disciplines to provide workers, clerical 
and administrative staff,
community agencies and others as appropriate.

As the Program Manager, the Nurse Practitioner provides 24-hour 
management, supervision
and leadership for a clinical program. The Program Manager: 1) 
contributes to forecasting
requirements necessary to long-range program planning and development 
of nursing and
related programs for effective delivery of health services; 2) 
designs, modifies, and
implements the cost-effective use of resources; 3) sets program 
standards for clinical
practice, administration, education and assures their implementation; 
and 4) promotes the
career progression of nursing staff within the program. Executes 
position responsibilities
that demonstrate leadership, experience & creative approaches to 
management of complex
client care.

Will work with the Medical Director and assist with clinical and 
administrative duties of the
program. the Program Director will coordinate home visits of all HBPC 
staff, monitor and
provide guidance to ensure all regulatory standards are met, review 
all referrals to the
program, coordinate and attend all treatment team meetings, provide 
backup, supervise the
staff RNs in the program and make supervised home visits to ensure 
the competency of the
staff RNs, provide assistance and guidance to the Administrative 
Assistant to ensure that all
regulatory and quality reports are done in a timely manner. The ARNP 
is responsible for
identifying the patient's medical problems, defining the medical 
management of these
problems, determining the need for consultation from 
medical/surgical/psychiatric sub
specialty clinics. The ARNP will consult and collaborate with the 
Medical Director and other
team members.
