Nurse Practitioner - On-Site Chronic Disease Management

HealthSTAT, a leading provider of ON-SITE Primary 
Care, High Health Risk and Disease
Management Services for employers throughout the 
United States, seeks Nurse Practitioners
in Dalton, Georgia.

This is an exciting and unique opportunity for a 
Nurse Practitioner to impact the lives of
employees of a major Northwest Georgia employer. 
This healthcare delivery model allows
NPs to utilize their skills, education and 
training to implement individualized plans of care for
employees and act as a healthcare coach for 
individuals with chronic conditions.

The Nurse Practitioner will facilitate chronic 
disease management, offer basic primary care
services and provide patient education to 
employees of a major Northwest Georgia employer
at on-site chronic disease management and primary 
care clinics. The mid-level provider will
diagnose, treat, prescribe, refer and manage 
health issues of employees while working
under the supervision of an area physician. This 
healthcare delivery model affords the
provider the autonomous means to exercise their 
skills developed through education,
experience and training to truly impact the 
health and well-being of the patients in a personal
and meaningful way.

Services are delivered in on-site locations and 
directed by the overseeing physician with a
pre-determined set of practice protocols and 
guidelines. The NP will not be responsible for
managing or treating Worker's Compensation cases.

Individuals interested in this position must meet 
the following qualifications:
* Ability to build strong relationships with patients and area providers
* Willing and able to work a schedule that will 
NOT require “on call” hours, but will be
structured to meet the needs of employees working 
in a manufacturing environment
* Completed usual advanced Nursing education with 
training in the diagnosis and
management of common medical conditions
* Current license in the state of Georgia and board certification
* At least 3 years of professional experience
* Wellness-minded
* Computer literate with ability to utilize 
electronic medical records to maintain notes.
* Ability to work collaboratively with the 
healthcare team which includes, but is not limited to,
Physicians, Nurse Practitioners and Occupational Health Nurses
