As one of the nation's largest, private, teaching health care 
providers, serving all of Delaware
and portions of Pennsylvania, Maryland and New Jersey, Christiana 
Care is currently seeking
a Heart Failure Advanced Practice Nurse for a full-time position 
available at Christiana
Hospital.

This critical role will function in an advanced nurse practice of 
adult medical patients with
the diagnosis of congestive heart failure, cardiomyopathy, transplant 
and/or mechanical
assist devices. Responsibilities will include providing care, 
education, consultation, and
support to clients (individuals, families, and groups), emphasizing 
the promotion of health
and the prevention of disease, economically and effectively. The APN 
will plan and
implement interventions in collaboration with other members of health 
care team, and
evaluates patient responses. In addition to serving as a clinical 
expert to hospital staff,
participating in local and regional professional and community 
education, and participating
in cardiovascular research, this position also supervises the 
Christiana Care Heart Failure
Telemonitoring program, and functions as a liaison to facilitate 
inpatient to outpatient Heart
Failure management.

To be considered, candidates must possess a Master's degree in 
Nursing and APN licensure
in the State of Delaware. ACLS certification is required. At least 
one year as an APN. Experience
with heart failure patients is preferred.
