Nurse Practitioner

Eaton Family Care Clinic, a Banner Health owned facility, has an 
opportunity for an
experienced Nurse Practitioner. Eaton Family Care in Eaton, Colorado 
is located just a few
miles north of Greeley, Colorado. The clinic opened in 2005, offers a 
broad scope of services
including acute, general and preventative care, geriatrics, 
pediatrics, laboratory services,
womens' healthcare and admitting privileges at North Colorado Medical 
Center (NCMC) in
Greeley. NCMC is a 398-bed regional medical center, Level II Trauma, 
Level II Nursery,
serving northeastern Colorado, western Nebraska, southeastern Wyoming 
and western
Kansas. NCMC provides the only Burn Center and air medical transport 
in Northern Colorado
(North Colorado Med Evac) and has over 350 physicians representing 
numerous specialties
and subspecialties on staff. Service area is 272,281. In 2005, a $98 
million expansion was
completed with focus on Cardiovascular and Surgical services.

The Nurse Practitioner will provide consults, follow-up and limited 
diagnostic services as
needed at NCMC.

Requirements:
* Credentials as a Nurse Practitioner, licensure as an RN and 
BLS/ACLS certifications
* Must possess or able to obtain a NPI, Medicare and Medicaid number
* A minimum of five years' RN experience, desire to work in a family 
practice, small town
setting and excellent computer and communication skills preferred
