Community Health Network is seeking experienced nurse practitioners 
to support our
school-based clinics

Duties: Develop tools to assist in patient care management to include 
education resource
materials for patient, family, and staff. Participates in clinical 
development of the wellness center.
Coordinate patient care activities between all caregivers and 
collaborating physician.

Experience: Demonstrated expertise in the practice of nursing as 
evidenced by at least two (2)
years of experience as a family nurse practitioner. In particular, 
looking for a candidate with
experience in triaging and caring for patients at a school based clinic.

Education: Must have a Masters of Nursing Science (MSN) from an NLN 
or CCNE accredited
school of nursing with preparation as an ANP.

License/Certification: Must be a currently licensed registered nurse 
in the state of Indiana.
Nurse Practitioner authorization or eligible, if involved in 
prescriptive behavior.

Skills: Candidate must have a genuine interest in delivering direct 
patient care; Candidate
should have both experience and a desire to focus on student health 
concerns; Candidate
should be capable of providing and leading educational sessions for 
patients, family, staff,
and physicians

Additional Information: Positions are located in Indianapolis, IN.
