Practitioner to join our team in the

provide a holistic approach to
caring for our aging residents in long-term care facilities. You

Key responsibilities of the Nurse Practitioner include:
* Providing direct care to aging residents in long-term care facilities
* Collaborating with primary care physicians, facility staff and 
patients' families
* Participating in care conferences with other providers and patient 
family members
* Working with the facilities utilization review process
* Identifying patients' behavioral health issues
* Educating nursing home staff, physicians and administration
* Coordination of care for patients

Requirements:
* Nationally certified Nurse Practitioner status
* Certification in Adult, Family or Gerontology
* Licensure by the State of WA
* Proficiency in Computer Skills

In addition, assets that would be beneficial include:
* Clinical experience
* Long-term care experience
* Knowledge of mental health issues
* A background in managed care
* Familiarity with Medicare and ICD-9 coding
