Nurse Practitioner*

Swedish Visiting Nurse Services (SVNS) is actively seeking a Nurse 
Practitioner to
join our team. The Nurse Practitioner is responsible for providing 
Face-to-Face
encounters for hospice patients and completing the certification for 
terminal illness
consistent with regulatory requirements. In addition, the Nurse 
Practitioner will
assist with the development of the nurse practitioner program, help to 
improve the
clinical excellence of the RN Case Managers and help with the strategies 
to expand
our services to other patients in the community.

A Master's Degree in Nursing is required. A current WA State Medical RN, 
a WA
State Medical AP License and Federal DEA Registration are also required. A
competitive candidate will demonstrate effective interpersonal skills, 
communication
skills in a diverse population, the ability to work effectively with the 
physical and
emotional development of all age groups and have experience in chronic care
management.
