Position currently available for rounder in skilled nursing facilities.

Job includes management of inpatients admitted to SNF for rehab after ortho
procedures, management after CABG or other cardiac diagnoses, continued
management of chronic conditions like pain, delirium/dementia, renal 
failure, UTI's,
wound care, IV hydration/antibiotics. There is a multidisciplinary team 
working in
each facility. One facility is a subacute unit: short stay for intensive 
medical
management. The other facility has a combination of acute short term 
patients and
long term care residents,patients who are on palliative care/hospice 
service, and
