**Nurse Practitioner or Registered Nurse -- Palliative Care**
**Full Time, Days*

In this position, you will provide education and facilitation for 
facility system
development to promote appropriate palliative care for patients and 
their families
who are living with advancing chronic illness.

You will assess the patient to identify palliative care (PC) needs; 
create and modify
a plan of care; make appropriate referrals; provide patient and family 
education;
monitor patient status; and evaluate the effectiveness of the 
established care plan.

Requirements for a Nurse Practitioner include Master's Degree in Nursing or
Science; current CA NP license; current CPR certification; National 
certification
within 1 year of hire in clinically applicable area (advanced practice 
geriatrics, or
hospice and palliative care management), must maintain national 
certification for
duration of employment; and documentation of a minimum of 30 hours every 
two
years of continuing education.

Requirements for a Registered Nurse include graduation from an accredited
nursing program; current CA RN license; 5+ years experience in an acute 
palliative
care setting or hospice; education or consultation experience; and 
expertise in
palliative care, advanced care planning and pain and symptom management.
