We are seeking a medical evaluator to conduct comprehensive health 
assessments
of our high risk Medicare population. These assessments include a 
face-to-face
visit where the member resides (at home or long-term care facility) or 
clinic setting.
The assessments help to identify candidates for disease and case management
interventions and are also shared with the member’s primary care 
provider to
address any medical follow-up needed.

Qualifications include two years’ experience as a family or adult NP. Board
certification and licensure are mandatory.
