*FNP or Geriatric NP –Elliot Senior Health Primary Care Southern, NH*

The Elliot Senior Health Primary Care Center in Londonderry seeks either a
geriatric nurse practitioner or a family medicine nurse practitioner 
with experience
in nursing home coverage interested in joining our outpatient healthcare 
center
dedicated to helping seniors to achieve their maximum possible quality 
of life.

This practice is a full service outpatient healthcare center that 
provides a seamless
network of healthcare services to the greater Manchester area’s senior 
population.
The care we provide extends beyond the exam room. We follow our patients at
select assisted living, nursing and rehabilitation facilities in the 
area. We believe it
is important for a senior to have consistent care from a provider they 
know, and
trust.
