(Convenient to
West Houston, Texas)---Minimum of one year experience as a Provider 
required
(Prefer Family Practice / Urgent Care Background)*

We are seeking a Family Nurse Practitioner or Physician Assistant to 
join a family
practice / express care clinic in Cypress, TX. The Provider would work 
10 hour shifts
during the week (Monday / Tuesday) and 8:00am -8:00pm Saturdays and 10:00am-
6:00pm on Sundays (every other weekend). The Provider would be working 
opposite
the other NP in a rotating shift schedule, so the Provider would also 
work Wed/
Thursday the opposite week and the other Provider would take weekends. An
example is listed below as Provider A and B:

A -- Mon/Tues
B -- Wed/Thurs
A -- Fri/Sat/Sun
B -- Mon/Tues
A -- Wed/Thurs
B -- Fri/Sat/Sun

The Provider would have on-site physician collaborative support during 
clinic hours
as well as a Medical Assistant and RN. The clinic has on-site X-Ray, Lab 
and EMR.
The base salary range is $50-$55 per hour, plus a $5,000 signing bonus, 
full benefits
package including Medical, Dental, 401K with match, malpractice 
insurance coverage,
CME and CME allowance. A relocation package is also part of the package 
and pays
$5,000 (reimbursed with receipts). Great location convenient to West
