*PNP Transitional Care*

The pediatric nurse practitioner with an interest in the chronic care of 
ventilator
dependent and medically complex patients is a great fit for this 
position in an 18
bed Transitional Care Unit. Major focus of this position is a 
patient/family advocate
for children transitioning from the Level IV NICU to a long term care 
facility. Advanced
nursing and leadership skills required for this full-time 40 hour week 
position with
rotating shifts. MSN, current Ohio RN licensure, APN certification and 
pulmonary
