Front Range Hospice is looking for a Nurse Practitioner for a Per Diem 
position. This position
will focus on F2F re-certification visits within our territory, which is 
North of Fort Collins to I-70.

The Per Diem Nurse Practitioner is employed by the hospice agency for 
the sole purpose of
providing face-to-face encounters. The scope of the nurse practitioner's 
expertise is to gather
clinical information to support the diagnosis and prognosis and provide 
these findings to the
physician who is recertifying the patient for hospice eligibility.

A qualified candidate must be licensed in Colorado and

Position Requirements:
- Prior Nurse Practitioner experience, preferred
- Hospice experience a plus but not necessary
